It focuses on older people (over 65) with multiple chronic conditions, a growing number of whom are living with chronic diseases across the European Union (more than 50 million) and who constitute a significant cost to healthcare expenditures. This group is often disadvantaged, living in rural and remote areas, or facing age-based discrimination.
